Researchers in India have carried out a data mining exercise to determine which are the most important risk factors in increasing the chances of an individual suffering a heart attack. Writing in the International Journal of  Biomedical  Engineering and Technology, they confirm that the usual suspects high blood cholesterol, intake of alcohol and passive smoking play the most crucial role in "severe", "moderate" and "mild" cardiac risks, respectively.

Subhagata Chattopadhyay of the Camellia Institute of Engineering in Kolkata adds that being male aged between 48 and 60 years are exposed to severe and moderate risk by virtue of their age and gender respectively, whereas women over 50 years old are effected by mild risk in the absence of the other factors.

Medical prognosis is a highly subjective art as is determining risk for particular health events, such as heart attack. After all, clinical history, symptoms and signs rarely follow a linear path and their interpretation at the individual level by doctor does not usually conform to the rules of epidemiology - personal intuition, emotions, logic and experience all conspire to confound the conclusion drawn for each patient at a given time under a particular set of circumstances.

The use of computational data mining techniques that allow researchers to extract interesting and meaningful information from real-life clinical data could remove at least some aspect of the subjectivity of clinical prognosis and allow the epidemiology to work at the patient level more precisely. There have been data mining approaches tried before. However, they often have inherent problems in that the classification of the data for information retrieval is based on decision making learnt from examples set by doctors and so they incorporate the very subjectivity that Chattopadhyay hopes to avoid with his approach.

He has used 300 real-world sample patient cases with various levels of cardiac risk - mild, moderate and severe and mined the data based on twelve known predisposing factors: age, gender, alcohol abuse, cholesterol level, smoking (active and passive), physical inactivity, obesity, diabetes, family history, and prior cardiac event. He then built a risk model that revealed specific risk factors associated with heart attack risk.

"The essence of this work essentially lies in the introduction of clustering techniques instead of purely statistical modeling, where the latter has its own limitations in 'data-model fitting' compared to the former that is more flexible," Chattopadhyay explains. "The reliability of the data used, should be checked, and this has been done in this work to increase its authenticity. I reviewed several papers on epidemiological research, where I'm yet to see these methodologies, used."

Types of Corrective Contact Lenses


Soft Lenses

Soft contacts are made of flexible plastic that is soft and permeable (allows oxygen to pass through the lens in order to reach the cornea of the eye). Routine Eye Exam

A routine eye exam exists for the purpose of detecting vision problems, eye disease and general eye health issues. What to Expect

The purpose of Day in the Life video (DITL also known as ADA video) is to maximize recovery potential for the injured client. DITL video is damage video. The production is most effective when it introduces the victim to the audience; reveals the human dimension of the injury; focuses on the damages; shows the injured client's life-altering functionality; expresses a sense of need and a sense of hope.

DITL video is used in focus groups, mock trials, settlement conferences, mediation proceedings, trials and hearings. Any personal injury case may require or benefit from DITL video: auto accidents (demonstrate that the client meets or exceeds the injury threshold); work related injuries, product liability; premises liability; medical negligence and toxic torts.

Depending upon the kind of coverage, DITL video may last from 3 to 20 minutes. Comprehensive coverage, which may also comprise interviews, demonstrations, deposition, testimony, documentation, records, photographs, home video and clinical examinations, can run from 12 - 30 minutes. Focused DITL which is unvarnished coverage edited for the potential of objection - usually depicting the injured client in his/her environment - runs for 7 - 10 minutes. DITL video can be incorporated into a Settlement Brochure that is not seen by the court. Settlement video is distinct from DITL video in that it is advocacy video which is unrestrained by the rules of evidence and caveats on prejudice imposed by the court. Settlement video is designed to bring out emotion and sympathy from the audience and is primarily used in settlement meetings.

For DITL video attorneys want high quality damages: evidence of strong probative value that meets the threshold of admissibility in court. Attorneys want similar quality in witnesses presented in the video. Attorneys want the jury to understand how gravely the injured client's life has been altered and they want the jury to make a human connection with the injured client and to perceive the injured client's sense of loss and also his/her sense of hope. In many cases it will be the jury that decides how much money will help the injured client. You definitely do not want to portray a cynical client who acts out of a sense of hopelessness.

Demonstrated damages might go beyond compensatory damages: associated injury is fair game in a DITL video and most especially for settlement brochures. The experts who appear in the video can further extrapolate on the scope of damages and testify to the injured client's potential care needs.

Insurance Adjusters look at the same elements that concern attorneys: the quality of damages, the quality of witnesses, how the jury will feel about the injured client and the associated injury and types of damage. The more effectively these elements are addressed in the video the greater it contributes to recovery potential.

The jury will also look at the quality of the damages. They need to decide how much money is necessary for the injured client's maintenance and recovery and whether the award will make a difference. The jury is concerned about the credibility of the information and witnesses they received and how responsible and dedicated the caretaker would be with the money they award.

There are three things that concern the judge and the Court of Appeals: Were the rules of evidence properly applied? Did the evidence have probative value that did not unduly prejudice the jury against the plaintiff or the defense? Is the damage award fair?

In this article I have discussed the purpose of Day in the Life (DITL) or Activities of Daily Living (ADL) video, what it must address and who its audience is and what they are looking for. I have also distinguished it from Settlement Video in this discussion. Forthcoming in this series I will discuss the actual video production.

Post Acute Withdrawal Syndrome is a debilitating condition that affects drug addicts and alcoholics in recovery. In fact, the symptoms of PAWS are widely considered to be the leading cause of relapse - especially in people who have been clean for several months or more. Understanding this condition is a critical part of mitigating the dangerous public health concern that is drug addiction and alcoholism.

Post acute withdrawal isn't a topic of particular interest in the addiction sciences at the moment; however, the condition has already developed a number of different names. These include the common acronym of PAWS, as well as Protracted Withdrawal, Post Withdrawal, Post-Acute Withdrawal and others.

What is PAWS?


PAWS symptoms generally set in after an addict or alcoholic has ceased using and gone through the physiological process of withdrawal and AWS or Acute Withdrawal Syndrome. AWS lasts anywhere from several days after the last use of drugs or alcohol, and can continue for up to 3 weeks in some rare cases. In general acute withdrawal symptoms subside after 7-10 days, after which time post acute withdrawal sets in.

Theory

Humans have an evolutionary trait that at one time ensured our survival, while today often backfires in the form of various addictions. This trait consists of a reward system whereby the brain releases "feel good" chemicals when a person does something that can ensure their survival; eating, sex, finding warm shelter, etc. As these chemicals are released the brain creates contextual reference points that serve to reinforce the "survival" behavior via a literal compulsion in the person to seek out those contextual associations.

Unfortunately, using drugs and/or alcohol mimics this process, leading to addiction when the user is compelled to recreate those events or feelings again and again. These behavioral processes are neurological in nature and are permanent. This means that even after a person has stopped using, neurological pathways in the brain continue to exist that constantly seek to recreate the "good feelings."

Post acute withdrawal essentially refers to the symptoms and behavioral changes and compulsions related to these neurological pathways. In lay terms, the user is still going through withdrawal, but the symptoms are no longer considered to be "acute."

Symptoms

Protracted Withdrawal symptoms are different for everyone. They can include: headaches, nausea, exhaustion, restlessness, night sweats, difficulty formulating and expressing thoughts, feelings of inadequacy and isolation, depression, mania, anger and rage, insomnia, suicidal thoughts, poor physical coordination (sometimes referred to as Dry Drunk), inability to solve simple problems, short and long-term memory loss, and many other types of symptoms.

Some people may only experience one or two of these symptoms, while others may experience many of them. This is partly dependent on attitude, physical fitness, mental acuity and a number of other factors.

Duration

Some people in recovery indicate that they've never had any signs of PAWS, while others state that they've had symptoms for years at a time. In general, the more severe a person's drug use, the longer the duration of active drug addiction and the higher the frequency of relapse, the more likely it is that symptoms of post withdrawal will continue for months or even years, and the more likely it is that those symptoms will remain consistent.

However, some people who only used drugs for short periods of time or those whose substance abuse problems were not considered severe may still experience pronounced and lengthy periods of protracted withdrawal symptoms. Conversely, some people who used heavily experience only light issues related to PAWS.

Overall, post acute withdrawal syndrome is a highly individual condition, but the primary unifying symptom is always the same: an acute, powerful and compelling urge to use drugs or drink alcohol again. This is because in essence those neurological pathways mentioned earlier are doing their job by causing the recovering addict to be compelled to reactivate, feed and thereby keep those pathways "alive."

Barriers to Public Acceptance

Many Americans are resistant to the concept of addiction as a disease, although most of the healthcare industry treats it as such on the surface. This type of attitude extends to PAWS in the sense that if addiction isn't a true disease, a secondary condition as a result of addiction probably doesn't warrant that much attention. It is precisely this type of thinking that has led to the relative obscurity of PAWS despite its obvious effect on people in recovery.

Treatment

Treatment for post-acute withdrawal has two primary objectives: to prevent symptoms before they occur, and to treat them individually when they do. For emotional symptoms of the condition this can include treatments like biofeedback therapy, individual counseling, group or family counseling and in some cases medication. For physical symptoms over the counter medications are a typical response, however, few people understand the critical role of diet and exercise in PAWS symptom management.

Practicing gratitude meditation on a regular basis is the best way to move towards happiness. Why? Because unhappiness is actually characterized as a state of desire or want. When you move out of that state by focusing on recognizing the blessings that you do have, you can move towards contentment. It becomes possible for you to understand that you don't actually need material possessions or any of the other things you desire in life, to be happy. Gratitude meditation is one of the simplest ways to teach yourself to be happy and experience fulfillment in the absence of desire.

Depression is a mental illness that causes psychological distress to the person suffering from it. It is generally described as a lowering of mood for a period of two weeks or more. This impairment can cause the victim much loss of normal physical and mental functioning and can be triggered at any time. There are causes such as distressing life events that are thought to trigger depression. Depression can fall into a number of categories depending on symptoms present. It can affect anyone at any stage in their life and does not discriminate against race, gender or socio-economic status.

Depression can be triggered by a life event. Depending on the nature of the life event, the length of time that the depression lasts for can be longer than 2 weeks, with some episodes taking longer than 8 months to resolve. Many life events can cause a depressive episode such as relationship difficulties, financial problems and health issues. The risk factors involved with depression are correlated with increased chance of suffering an episode, so its important to educate the public and bring awareness to individuals.

Being a psychological disorder depression affects the mind, and thus can affect a person's ability to function effectively in society. Depression can have take on three distinct forms, often going unnoticed by people not suffering from the disorder. Depression can affect a persons cognitive abilities, work performance and ability to socialise with others. Loss of interest in activities is also common which before the onset of the depression gave much pleasure, and the withdrawal of pleasure from these activities is a sure marker that something may be wrong.

The seriousness of depression can be dependent on many factors. A persons genetic history is a strong indicator, and if any family members have been diagnosed then the chances of depression are increased. The biological make-up of an individual also plays its part, with some people more prone to depressive episodes purely because of individual biology. Socio-economic factors such as lower incomes, drug and alcohol abuse and social isolation and vulnerability all play a role.

Depression affects both men and women, all ethnicities and adults and children. 1 in 4 women will experience depression, while 1 in 6 men will go through an episode at some point in their life. Due to the stigma and the taboo nature of depression, this mental disorder has gone relatively unnoticed in the past. With help from media attention it is now receiving much needed public attention.

There are three major types of depression that are diagnosed by mental health professionals.

Non-melancholic depression or clinical depression:


This is the most common form of depression. The nature of non-melancholic depression is thought to be psychological in origin, which is in stark difference to the other less well known and rare forms of this mental illness. This is a difficult form to diagnose as the boundaries of its symptoms are not well defined as opposed to the two other forms of depression which are discussed next.
Some common symptoms of clinical depression are:

- a depressed mood extending for a period of more than two weeks

- loss of interest in activities they were previously pleasurable

- low, miserable and/or sad feelings remaining through-out the day

The second less well known, and even less diagnosed form is melancholic depression. This form in contrast with clinical depression has its origins in biological factors. Individuals suffering from this illness are a minority with only 1 - 2% being diagnosed.
Common symptoms are:

- severe sleep disturbance

- marked changes in appetite and weight gain or loss

- changes in cognitive performance such as memory, concentration and problem solving.

- distinct loss of pleasure from activities that were previously found to be pleasurable

The third form is psychotic depression which is the most rare form of depression and is characterised by symptoms that share similarities with severe clinical depression and schizophrenia including:

- extreme depressed mood and loss of interest in pleasurable past-times

- hallucinations, psycho-motor problems and delusional thinking

- severe sleep disturbances

Depression is a wide-spread mental illness that may be affecting a loved one, friend or work colleague. On outward appearances it may be hard to tell if someone is suffering from depression. Certain risk factors are responsible for determining if a person will suffer from depression with women being at higher risk than men. What is CBT?


CBT, or cognitive behavioural therapy is a psychotherapeutic e.g. (treatment) approach to helping people fix emotional, cognitive and behavioural dysfunctions. A trained medical professional helps a patient to restore their quality of life using CBT, to change a patients thought processes, coping responses and behaviours.

How does CBT work?


CBT works by replacing undesirable thinking patterns with patterns of thinking that help the individual to cope with their problems. Thinking patterns such as over generalisation, magnifying negative thinking, and minimising positives are all examples of destructive thought patterns.

What techniques does CBT use:


CBT practitioners use a number of techniques:

Exposure and response reduction therapy - exposure to a stressor such as dirt in an individual that fears dirt, and then helping them to manage that stress response. This is relevant to people with obsessive-compulsive disorder.

Stress inoculation (meaning that the individual is more resilient to stress)

Cognitive therapy (therapy aimed at memory, thinking skills, problem solving and decision making)

Acceptance and commitment therapy

The process of undertaking CBT:

1. Assessment - A qualified medical professional prescribes an examination to determine the best treatment plan.

2. Conceptualisation - re-framing the problem issues in a different way.

3. Learning new skills such as self-talk, exposure response, relaxation techniques and minimising negative thinking.

4. Skills consolidation and application training - Learning to apply the skills in practise and then using them in the real world. E.g.: talking to a stranger or attending a social event.

5. Generalisation and maintenance

6. Post-treatment assessment follow-up
